TEAM is all about finding the potential to make positive changes and healthier choices!
The program includes:
- Weekly online learning sessions
- Weekly phone coaching calls with a health professional
- Resource packs including handouts, recipes, equipment and a Garmin activity tracker
- Weekly targets for adolescents to work towards
- Weekly SMS and email reminders
- Pre and post program surveys
- Downloadable GP report
- An end of program reward voucher up to the value of $60
After completing the online session, adolescents together with their parent or carer, will speak to their TEAM program coach at their scheduled weekly appointment time and discuss how they are progressing towards achieving the weekly targets.
Adolescents are eligible if they are:
- Aged between 13 to 17 years
- Living, studying or seeing a health professional within the Central and Eastern Sydney area
- Given informed consent by a parent/carer
And meet one or more of the following criteria;
- Classified as above a healthy weight*
- Diagnosed with a chronic health condition, or
- Score 10 or more on the TEAM eligibility screening questionnaire

Unfortunately, adolescents who do not meet the eligibility criteria are not able to participate.
*Based on clinical growth charts by Centers for Disease Control and Prevention (>85th percentile for age and gender)
Throughout the program there are various targets to complete. Some of these targets will only require slight changes to your daily habits and others may require more time and planning. The targets chart received before the program outlines all the targets set for the program, so there is plenty of time to plan the activities.
Your TEAM coach will be able to support you and provide advice to help you get back on track.

You will be required to complete a medical questionnaire at the start of the program to inform us of any existing medical conditions.

Our programs are delivered at a broad population level. Whilst the programs do not specifically target Aboriginal and/or Torres Strait Islander families, our program facilitators complete culture competency training to understand and be considerate of personal beliefs, culture, and individual circumstances when supporting families throughout the programs.
BHCo continues to look at further opportunities to review the structure and content of our programs to best support all Aboriginal and Torres Strait Islander communities.
